Mini Dental Implants What would we do without them? Go around without teeth?

It seems like all the patients that need to come see the dentist wait until the very last minute to ask for help.  We really are not that bad here at Lake Norman Mini Dental Implant Center. 

My next patient had a Cone Beam 3D scan done in March of this year.  She had a failing root canal on one of her upper front teeth.  She must talk to some of my other patients because she had done the same thing as several others…..super-glued her post and crown in at least a dozen times.Images[5]  Should I just consider superglue to sell here at the office?

 

All kidding aside…I must say its pretty creative what patients will think up to solve their dental problems.  I know it takes a lot  of courage to come into the dental office.  Those patients who are deathly scared of us please know that we are here to help.  We will not judge.  We only offer solutions and a range of them to fit what your goals are.

On her 3D CBCT we can see the HUGE defect in the bone where the dental abscess is at the tip of the root canal tooth.

And this is from March…its now December…hopefully I could thread in a Intra Lock 2.0mm mini dental implant into the Palatal(roof of the mouth side) bone to hold a temporary crown.

 

 

I took the tooth out preserving as much of the bone as possible.  It was now time to try and get the Intra Lock Mini Dental Implant in.Defect w mini in place

 

 

 

 

 

 

 

The Intra Lock mini dental implant was rock stable (over 40ncm) and now we would pack the defect with some bone grafting material and a collagen membrane.  In a few months we should have good bone to work with.  My initial thoughts are that the mini is going to integrate(heal) into the bone.  If it does not we should have enough new bone to place a new mini implant.

Broken Tooth ~ Extraction ~ Dental Implant Procedure Upper Tooth

My patient who alternates between our office and the periodontist office came in today.  She had been told a few months ago by the periodontist that her upper canine tooth crown was failing and would come out soon. But as most patients and people are we hate to go see our dentist until…..it breaks

Broken upper canine tooth.  She had super glued the post and Porcelain fused to Metal crown to the root for the weekend.  Not a great solution as you can see from the gum tissue.  But I thought it was quite creative.

To get the rest of the root out without damaging the delicate plate of bone on the cheek side I had ordered some SalvinPDL-elevators.  These are thin bladed instruments used by the dentist to remove the broken root without damaging the bone.  Thanks Chris Totty for sending them out.  I had another brand but needed another set and I quite like these.  Plus Salvin Implant Supply is here in Charlotte NC.Salvin pd-Elevators

Canine root being removed and as much bone being preserved as possible for an OCO Biomedical ISI 4.0mm dental implant placement.

Now with the canine root out I could check the quality of bone and pick what dental implant would make the most sense to place.  We could let the area heal and place a few mini dental implants or consider a one piece OCO Biomedical Immediate Stabilized Implant.

Having a stock of OCO Biomedical Implants that Matt Baron and Chris Sanchez had sent me I quickly found a OCO ISI 4.0 x 14mm that would work great in a situation like this.

Following the simple OCO dental implant placement protocol I used the pilot bit to ensure the correct angle was being followed and took an xray to confirm.

OCO Biomedical ISI 4.0 x 14mm dental implant in the extraction site.  Notice the gold color Titanium Nitride finish.  This will give a good warm color to the implant and final crown under the gum tissue.

I used her old PFM crown as a temporary and fit it right over the OCO ISI dental implant.

I think this case could have been solved either with a mini dental implant capturing the palatal slope of bone or a traditional implant as shown.  I have done both and they both will work well with the proper training.  I am encouraged by the number of dentists who are now seeking to get dental implant education.  There are several great mini dental implant seminars and education as well as regular implant CE courses.  I encourage any dentist to get started and mini dental implants are a great way to move forward in helping our patients.

Extraction and Immediate Placement of OCO Mini Dental Implant

What are mini dental implants good for?  I would say lots of things; like immediate use, less cost than a regular dental implant,less trauma, less pain, less healing and more immediate satsifaction for both me as the dentist and the patient. 

Before OCO i-mini

 

Before a one piece mini dental implant, like the OCO i-mini and OCO i-micro existed, I would have been tempted to do some heroic dentistry.  In this case a post, crown lengthening and a porcelain crown.  In a few years this patient would be back in my dental chair with my crown in her hand and we both would be frustrated.

Clinical view of the broken lower front tooth.  In this case even if I wanted to do a bridge it would have been a bad idea.  The adjacent tooth (the one on the left side of the broken tooth) was part of her bridge that extends back to the molar.

 

 

 

 

So off to the OCO Biomedical tool-chest of goodies that Chris Sanchez sent me.  I found a 3.0 x 16mm mini dental implant.  Ahh…..this would work well here.

With some Salvin Peritomes (sharp dental extraction instruments used to removed the broken tooth with minimal trauma) I proceeded to extract the root.Root extracted

With the tooth root out and the bone on both sides still intact I decided to continue with the OCO mini dental implant placement.

To be certain I was keeping in line with the adjacent teeth and roots I took a dental x-ray with the pilot bit in place.  Looks like we were in good shape to place the OCO mini dental implant.

Mini Dental Implants Case presentation of the week – Here is a patient that was referred to our office by another patient that we placed mini dental implants and restored the minis with crowns.

He had ALOT of needs.  But as with most things we see, I had some ideas and the patient had their own set of ideas.  With ample discussion and informed consent he decided to have a few extractions and some mini implants to support fixed crowns and bridges.
