Mini Dental Implant vs Regular Implant ~ Whats the difference?

I get asked this question a lot from both Doctors and Patients.  I think a better way to look at this is to compare one or two piece dental implants. 

Mini dental implants are one piece in design…the part that comes up into the gum is connected to the screw part that goes into the bone.  One piece means for the patient less expense.  But there are one piece dental implants in regular implant sizes.  OCO Biomedical has a one piece, 3.25mm + 4.0mm + 5.0mm.  Once again the same theme….one piece less expense for the patient.

So when would you use a one piece regular size implant?  I think when you have enough stability in the bone that would allow immediate loading.  Most of the time that will be in the lower jaw; but in this example we were able to do this to replace an upper missing molar.

My patient had a missing upper left molar.

Our plan was to replace it with a regular size implant (we could have done it with two mini dental implants, but he preferred to have a traditional size solution).

He had been missing that tooth for well over 7 years.  This had allow his sinus cavity to 'drop down' thus reducing the height of bone we had to place a dental implant.  Our plan was to do a 'sinus tent' and gain a bit of height by adding some bone through the small hole that was made in his gum.  Easy to do and very predictable.

OCO Biomedical ISI one piece 4.0mm implant was selected.  The bone was very dense and I was fairly certain I would get over 35ncm of torque to stabilize the implant.  If I did not then a two piece 4.0mm implant could have been placed and left to heal into the bone for the next 12 weeks.

Final Thoughts:  In a well designed implant system placing a larger size implant is just as easy as placing a mini dental implant.  With the OCO system the only thing you change is the final drill bit.  There are no other drill sizes to work your way through. 

What I like even better is the ability to now have either a one piece or two piece implant that offers the same prosthetic abutment.  If you do place a two piece OCO and put in a solid abutment you have now created a one piece OCO ISI implant. 

The other thing I like about these implants is that you cement the crown to the implant body, not the abutment, so the 'micro gap' in internal to the implant crowns.

Mini Dental Implants ~ Extractions ~ Lower Denture

My next patient is a great example of how versatile mini dental implants can be.

Lower ridge with 2 teeth left.  Patient has been wearing an all acrylic partial denture.  He is going to do a sequenced treatment plan where we place 4 mini dental implants and make a full lower denture.  As he is able to accumulate the funds we will add in 4 -5 more mini dental implants to support a full lower porcelain bridge.

The teeth are extracted and the mini dental implants are placed.  The only bleeding that occurred in this case is from the extraction sites; not from the mini dental implants.

This kind of placement allows patients to have less healing and less pain.

From here is will be a straight forward overdenture case.  We were able to get his first impression today after the mini implants were placed.  From that a wax rim will be made with some speaking wax to record how his jaws come together.  In a few weeks his denture will be complete; the gums where the teeth were extracted will have closed in and he will have a snap in/ snap out lower full denture all thanks to the mini dental implant.

Dental Implants to Replace a Missing Back Molar

Are you missing a back tooth?  Then you should consider getting a dental implant.  A dental implant acts as the root of the tooth and can allow for a crown to be placed over the implant.  No need to grind down healthy teeth on either side of your missing tooth for a bridge.

X ray of my patient showing the missing upper molar tooth.  One option would have been to grind away healthy tooth to create room for a bridge.  The teeth on either side of the missing tooth would have to be drilled on for a 'crown' and that would hold in the bridge.

With a dental implant we can save healthy tooth structure by not having to shape the adjacent natural teeth.  In some cases it can cost less to do a dental implant and crown instead of a bridge.

Clinical picture of the same OCO two piece dental implant with a healing cap.  This is the day of placement.  Notice the lack of blood or stitches or the gum having to be cut and pushed back.  This traditional size dental implant is placed in the same fashion as a mini dental implant.  Very little surgery, very little discomfort and post operative pain for the patients.  Because the bone in this case was very soft I decided to use a regular sized traditional two piece dental implant.

If the bone had been dense I could have placed multiple mini dental implants to support the molar crown as shown in this case.  Both solutions work and work well.  It is really a choice for the patient and dentist on what can be done and at what cost (economic, time, and potential discomfort).

 

 

 

This implant will heal for several months (12-16 weeks) before we can 'load' it with a crown.  In the case shown above with the mini dental implants because there was good dense bone I was able to place the crown over the implants in 3 weeks.  The mini implant crown has been in function now for over a year.

I will continue the post to show the abutment (the part of the implant that comes up out of the gum for the crown to attach to) and final porcelain crown.

BioHorizons One Piece 3.0 Dental Implant & Laser Lok Two Piece 3.0

For tight spaces in the esthetic zone where I normally would have placed a one piece 3.0mm dental implant, BioHorizons now has  a two piece Laser Lok 3.0 implant.

4 OCO crown and bridge head mini dental implants in place to anchor two molar size crowns.  A lot of times Dr.s ask me why are you putting in two mini implants to replace one tooth instead of one larger one?  My answer (best worded by my friend Dr. Chow) is lets replace the upper molar tooth by replacing it with the most anatomically and biologically correct position of the missing roots.  2 or 3 implants to disperse the force the way the body originally had it.

The 4 mini dental implants are inserted with 2 engaging the palatal(roof of the mouth side) slope of bone.  The other 2 engage the buccal(cheek side of the bone).  This way we avoid the sinus cavity and have primary stability from the day of placement.  This is the original positions of the roots the mini dental implant will replace.  These mini dental implants had over 40ncm of torque day of placement.

I often get asked questions on how can patients keep the crowns clean.  Each crown has a 'flossing groove' built into the anatomy.  This will help the patient guide the water pic or floss threader.

Intra Lock Mini Dental Implant to Replace a Missing Tooth with a Crown

With over 75 Million Americans missing at least one tooth I am amazed that there are not more mini dental implants being placed.

Typical of what most patients who are missing teeth have.  A empty spot somewhere in the middle of their teeth….just past the smile line.  If they smile big enough you see the empty space.  This is easily and painlessly corrected with 2 short visits to the dentist and a mini dental implant and crown.

In this patient case a Intra Lock MILO 3.0mm mini dental implant was placed from Shatkin FIRST.  No surgery, no flap of gum tissue was cut, no bone was added.  Just a simple procedure to gently rotate the mini dental implant into the gum.  A small dental bit was used to create a opening in the gum the size of a pencil lead. Notice the lack of bleeding or trauma to the surrounding gum tissue.

I decided to use Mini Dental Implants due to the thin ridge of bone that was left.  Once teeth are extracted the bone will melt away over time.  Adding a partial denture will accelerate this process by the constant rubbing on the gum tissue. 

This x ray shows the OCO Biomedical Implant in place.  Notice how thin the bone is.  If we did not have the option of using mini dental implants then either this case would not have been possible or the patient would have to have an additional surgical procedure to expand the bone or add a block of bone to the existing lower jaw.  Both of which will increase expense, time, healing and discomfort.

Ace Surgical Mini Dental Implant Full Arch Roundhouse Bridge

Can dental implants be affordable?  My next patient wanted to get rid of his dentures and have a fixed set of crowns or a full arch roundhouse bridge on dental implants.  He brought his treatment plan for 8 dental implants and a set of 12 porcelain crowns…..total cost a bit north of $28,000.  With mini dental implants we could reduce that cost and still have a well engineered bridge.

He had a nice ridge of bone to work with.  It would not need any pre surgery to accommodate dental implants.  With some planning with Jon Younce CDT at First Impressions Dental Lab a logical progression treatment plan was formulated that was restorative driven.

His pan x-ray also showed good bone with the sinus cavity totally dropped down where the back teeth were in the upper jaw.  There was also a retained set of root tips that would need to be removed on the upper right.

Using his upper denture as a guide I made holes in the center of each back tooth and the lingual of the front teeth to ensure the mini dental implants would line up correctly for the final roundhouse full arch bridge.

10 ACE Surgical Mini Prep 2.3mm implants were placed in the upper arch.  They were placed as parallel as possible to allow for proper draw of the final roundhouse full arch bridge.

His denture was soft lined to fit over the ACE Surgical Mini Dental Implants.

A regular crown and bridge impression was taken of the 10 ACE Surgical Mini Dental Implants.  No additional impression copings or lab analogs were used.  With mini dental implants we can create a more efficient and cost effective solution for patients by reducing parts and pieces that are normally required with traditional dental implants.

A standard crown and bridge model was poured in Pol-u-rock by the lab and a set of trial resin crowns were made to let the patient 'test drive' the crowns.

It was decided after a few days that the temporary crowns were too long. The patient also had Bell's Palsy and we decided to try and correct the slant.  This photograph shows one side of the temporary crowns shortened to the new lenght for the lab to see our corrections.  I shapedthe other side and let the patient go home to 'test drive' them again for final approval.
